Incident Narrative
An employee had just finished working on a fan motor. When the fan was turned on to verify the direction of its rotation, the vacuum of the fan pulled the tip of the employee's right index finger into the spinning blade, causing it to be severely lacerated from the middle of the nail to the end of the finger.
