Incident Narrative
A part jammed in the track of the thread roller on a piece of machining equipment. An employee removed the access cap to get the jammed part. Once the part was removed, the load shuttle moved horizontally and contacted the employee's right middle and index fingers. The middle finger was partially amputated.
