Incident Narrative

An employee was driving a stand-up dock stacker forklift to pick up a pallet. He attempted to avoid hitting a stack of tires by turning the wheel sharply and lost his balance. His left foot became dislodged from the forklift, while his right foot remained on the lift; the lift was therefore still in motion. His foot became pinned under the tires and the forklift.
