Incident Narrative
An employee was performing a belt replacement on a fan motor for the AC-1 unit. The employee had locked out the power source(s) to the fan motor, loosened the motor / tension on the belts, and replaced the belt when the pulley "free-wheeled." Her fingertip was caught between the belt and the pulley, resulting in a fingertip amputation.
