Incident Narrative

During a project to make way for a fire sprinkler system, employees were doing demolition work. While approximately 15 feet above the concrete floor, the employees worked from a 2x12x20 plank in the rafters. The injured employee was using a leaf rake to remove insulation when the employee fell approximately 15 feet to the concrete floor below, suffering a head injury.
