Incident Narrative
An employee was greasing a tree shear attachment on an excavator with an automatic greaser when the tree shear head rotated while the employee was out of the cab and the equipment was idling. The shear head caught the employee's work vest and pulled the employee's right arm into the shear head. The employee suffered a deep laceration to the underside of the right arm up to the armpit, requiring hospitalization and surgery.
