Incident Narrative
An employee had been watching the gears on the back side of a pleater, trying to determine what was causing a pleater to "jump" and not work properly. The cuff of her left glove was caught in the gears; as she tried to free it, the gears grabbed the tip of her right glove, pulling in her right index finger and causing an amputation to the finger. The machine was not guarded at the time.
