Incident Narrative

The injured employee and a coworker were moving a walk-behind powered pallet stacker down a slight decline from an elevated platform to a lower level to prep for moving additional equipment on the lower level. The stacker moved quicker than expected and the brake did not activate sufficiently enough to stop the stacker completely on the decline. The stacker continued to move down the ramp, causing the employee to let go of the machine handle. Once the stacker reached the flat surface it spun to the right, striking the injured employee on the right side of his right foot causing a broken right foot and a laceration.
