Incident Narrative

Employees were setting 5-foot by 5-foot concrete mechanically stabilized earth (MSE) wall panels. While they were adjusting a panel, the tooth of an excavator caught the metal strap of an adjacent retaining wall, causing it to fall over and break an employee's pelvis.
