Incident Narrative

On November 5, 2019, at 2:07 PM, an employee was walking between a tote container filled with empty hangers on the floor and a Z-rack. As she turned the corner, she tripped over the base of the Z-rack and her face struck the base of the Z-rack bar. She sustained a compound fracture to her left arm and a right wrist fracture, requiring hospitalization.
