Incident Narrative

An employee was unloading a box that was positioned on a pallet. The forklift driver came over to move the pallet. The forks did not go all the way in. When the pallet was moved, it shifted and pinned the employee's left leg between the pallet and a flatbed trailer. The employee suffered hyperextension of the left leg.
