Incident Narrative
An employee was outside of the store, at the loading dock, receiving a shipment of shopping carts. The carts were packaged on a pallet, handle side down. When the carts were separated, they shifted and pushed the employee off the loading dock. He landed on his left side, striking his shoulder, head, and hip.
