Incident Narrative

An employee was preparing to unload supplies to install a rooftop HVAC. The employee was pulling a crate of materials to the edge of the box truck so that a forklift could pick it up. The crate broke, causing him to fall off the back of the truck and onto the ground below, resulting in a pelvic fracture.
