Incident Narrative

An employee had a spool of wire on a spooling machine. The spool was wrapped with plastic wrap and the employee was taping it as it was spinning. While holding the tape with his left hand, he went to cut the tape but it didn't break off. The machine then pulled the employee, flipped him over the machine, and wrapped his arm around the machine. The employee sustained multiple fractures to their arm.
