Incident Narrative
A truck driver was delivering 30-inch PVC pipe. While the pipe was being unloaded from the rear of the truck and the driver was standing nearby, a pipe came out of the cradle in the front of the truck and fell off the truck. It hit the driver's left leg resulting in surgical amputation. The driver was hospitalized.
