Incident Narrative
The injured employee climbed up a ladder to access the top of a concrete ledge, which was part of the support structure for a furnace, and was standing on the north side of a cylinder as another employee was operating a lift to align the pin on top of the cylinder. A third employee was working a come-a-long to help align the cylinder top. The injured employee was flagging from the platform when he fell approximately 8 feet, landing on the concrete surface below. He was hospitalized with a fractured tibia and fibula in the left leg.
