Incident Narrative
An employee was loading a one-person aerial lift onto a trailer and securing the lift to the trailer with a come-along. A hook from the come-along came loose from its attachment point on the trailer. The tension from the come-along cable ratchet system caused the hook to fly back toward the employee, striking and injuring his left thumb resulting in medical amputation of the thumb tip.
