Incident Narrative
While an employee was observing and lubricating a chain-driven conveyor belt, a spray bottle he was holding slipped out of his hand. As he attempted to catch the spray bottle, two of his fingers contacted the moving conveyor belt resulting in the partial amputation of his index and middle fingers.
