Incident Narrative
An employee operating a tire shredder was using a pry bar to free a jammed tire from the tire shredder's conveyor guard. The employee's hands became caught between the pry bar and the guard. The employee sustained four fractured metacarpal bones in the left hand and the right thumb was amputated and later reattached.
