Incident Narrative

An employee was in-cutting a steel slab with a portable burning machine. The machine did not cut all the way through the slab. The employee bent down to turn off the machine and the end of the slab broke loose, causing the machine to come down and hit the employee in the back of the head. The employee was hospitalized, suffering from a laceration and bleeding.
