Incident Narrative
The injured employee was unloading a truck and staging materials while flagging/directing a forklift. As the operator shifted the load, the forks of the forklift bumped a steel beam that was staged on top of wood dunnage. The beam shifted and fell onto the injured employee's left foot. The employee sustained 15 fractures to his foot that required hospitalization and surgery.
