Incident Narrative

A forklift was used to lift a set wall, which weighed several hundred pounds, so an employee could shim it. The forklift blade was inserted beneath a metal beam that the 10' by 8' wall sat on, causing the beam to bend when it was lifted. When the employee attempted to shim the wall, his left pinky finger was crushed/pinched, resulting in a partial amputation.
