Incident Narrative
An employee noticed a coolant hose dripping on a blow mold during a new mold setup. The employee attempted to push the hose into a quick connect fitting to stop the leak. Upon reaching into the molding area, the employee's thumb came to rest on top of a positive stop. The mold then closed, capturing his thumb on the positive stop. His right thumb had to be surgically amputated. The blow mold was guarded at the time of the incident.
