Incident Narrative
An employee had climbed to the top of a ladder to stock tools. The wheels on the bottom of the ladder were not locked and another employee began to move the ladder to another location. The employee on top of the ladder lost balance and fell to the floor, suffering broken ribs and a punctured lung.
