Incident Narrative

Employees were prepping a vessel for painting. The employees were using a pressure washer and working from a scissor lift. Another employee who was moving in a scissor lift became entangled in the pressure washer line. The line was tied off to the lift with the employees who were pressure washing. That lift tipped over and fell onto a third scissor lift. The employee suffered a dislocated tibia.
