Incident Narrative
An employee was troubleshooting a testing machine and cleaning the laser. The machine has a rotating table above a hole where the laser shines through. The employee's left middle finger was caught between the edge of the hole opening and the rotating table when the machine was activated resulting in a fingertip amputation.
