Incident Narrative

A maintenance employee was troubleshooting a machine. A chain drive appeared to be stuck. When the chain freed itself, the employee's right ring and middle fingers got caught in the chain. The middle finger was crushed/fractured and the ring finger was amputated at the second knuckle.
