Incident Narrative

An employee was driving a utility terrain vehicle (UTV) and struck some 4-inch PVC pipes laying on the ground. The employee lost control of UTV and as he was about to be ejected he grabbed the rear view mirror causing it to break and lacerate his left index and middle fingers.
