Incident Narrative
An employee was ripping plywood into 2 1/2" strips with a table saw. He was toward the end of his rip cut when the blade hit a knot. His push stick and his right hand were pulled into the blade. He suffered cuts to the thumb and index, middle, and ring fingers; the index finger was cut through and had to be amputated at the second joint from the tip.
