Incident Narrative
An employee had just completed mixing the ingredients and was dumping the mix into a tote. The tote was placed under the ribbon blender and the two discharge doors were opened. The blender pushes the product out the discharge doors and into the totes. Towards the end of this process, the employee noticed a small pile of product inside the discharge door. She attempted to knock it into the tote, at which time, the blender rotated around and caught her right hand between the edge of the discharge door and the blender. She sustained crushing injuries to the hand and severe lacerations to the hand and index finger. An employee was cleaning a mixer in the grinding room and suffered a finger laceration.
