Incident Narrative
An employee was preparing a shell core machine to make shells. The employee installed the right half of the pattern and placed the left into position. The employee activated the machine to bring the two pattern frames together. The rotating cylinder became stuck and the employee hit it with a hammer. The frames then closed on his left ring finger amputating it below the first knuckle.
