Incident Narrative

An employee was using a roll of shrink wrap to wrap a skid of plastic lugs. He was using a plastic "handle" that inserts into the cardboard tube holding the shrink wrap. He was wrapping the skid from the bottom to the top. As he neared the top and was pulling the last wrap tight, the handle came loose from the roll of shrink wrap . He stumbled a few steps and fell to the floor. The employee was hospitalized with a fracture to the left femur.
