Incident Narrative
An employee was trying to bend the top rail of a rig mat frame using a pry bar so that he could install a filler block. While his right foot was inside the frame of the rig mat for leverage, the bar slipped, and the momentum caused him to fall to the ground with his foot still in the rig mat frame. His leg was broken above the ankle. He was hospitalized for surgery on his right leg.
