Incident Narrative
Employees were changing a gasket on a steam line, using a tubular welded scaffold with 28-inch aluminum work planks two sections high. An employee fell about 12 feet from the scaffold, making contact with the scaffold on the way down and suffering five broken ribs, a punctured lung, and a laceration to the forehead. A harness was being worn at the time of the incident, but had been unhooked.
