Incident Narrative

An employee was attaching/engaging a plate bug or clamp to a finished metal part weighing about 100 lbs. still within a press brake. The downward motion of engaging the plate bug onto the part caused the part to fall off of the press brake. The employee suffered an amputation injury to the right middle finger.
