Incident Narrative
A forklift got stuck in the loose gravel outside the facility. It began to fall over while pulling it out of the loose gravel using a yard truck. The forklift operator's left hand was crushed between the roll over protection cage and the ground resulting in the amputation of three fingers. One finger was reattached.
