Incident Narrative
An employee was getting ready to load a forklift with a 55-gallon drum of hydraulic fluid. While the employee was manually pushing the forks into the pallet, the forks shifted and his right little finger was caught between the forks and the pallet resulting in amputation of the fingertip.
