Incident Narrative

The injured employee was using a chalk line to mark the location of conduit under the roof membrane. The employee slipped on standing water/ice that was near the flag line and fell towards the warning line system. As the employee tried to catch himself, he fell approximately 17 feet off the roof to the ground below. The employee suffered a left hip fracture and lower lumbar fractures (L3 and L4 vertebrae).
