Incident Narrative

An employee was attempting to adjust a row of televisions on a metal rack while on a power lift. When he stepped off the lift onto the steel rack, he fell 20 feet to the concrete below, fracturing both arms, contusing his right eye, and lacerating/fracturing his skull/forehead.
