Incident Narrative
A bucket in a drag conveyor came loose and fell to the bottom of the drag conveyor. A temporary employee was attempting to retrieve the bucket when the drag conveyor energized, pulling the temporary employee's left arm into the conveyor and amputating it at the elbow.
