Incident Narrative
An employee was operating a machine that cuts penne pasta when strands of pasta became jammed in the machine. He attempted to clear the jam and removed the flexible plastic tubing. He saw that the pasta was jammed in the cone and tried to push the dough out of the bottom of the cone. The moving blade then amputated his index fingertip. The machine was not locked or tagged out at the time.
