Incident Narrative
An employee was removing a keeper pin with another employee when the keeper pin released more easily than expected and the pin removal tool slipped into the opening of the frame. The employee's fingertip was caught between the head of the pin removal tool and the turn table, amputating a portion of the fingernail.
