Incident Narrative

An employee was hunched over cleaning up debris from the floor using a small shovel when a nearby forklift started backing up. The forklift driver did not see the hunched over employee and ran him over. He suffered a broken left ankle and a fractured right ankle and foot, requiring hospitalization.
