Incident Narrative
An employee was putting away a pallet consisting of 16 cases of canned carrots in a warehouse. While unpacking the pallet of canned carrots, the employee lifted a case off the second to last row and a can of carrots from the bottom row exploded upward, striking the employee's face. The employee suffered facial lacerations, a left nasal bone fracture, a left orbital floor fracture, a soft tissue hernia in the maxillary sinus, left lateral rectus swelling, and loose teeth, requiring hospitalization.
