Incident Narrative

An employee was changing the shield on an aluminum pot. As he removed the shield, he tried to toss it to the floor, but the latch on the shield caught his glove, causing him to lose his balance. He then fell from the walkway between the pots (approximately 36 inches above the floor) to the floor, landing on his left shoulder, with the shield landing on him. He suffered a contusion to the left shoulder, laceration to the left hand, laceration to the left flank area, and abrasions to the left side of his face, requiring hospitalization and sutures.
