On July 11, 2018, an employee was mounting a 16-inch tire onto a 16.5-inch rim. The tire would not seat on the rim, so the employee used a 150-psi air line to inflate it. The tire and wheel assembly then burst and struck the employee, who suffered a broken arm and lacerations to the arm and face.
