Incident Narrative
An employee was painting a 35-foot trailer from beneath. The trailer had been lifted on two forklifts, one at the front and one at the back. The tilt cylinder on one of the forklifts had a slow leak and tilted forward, causing the trailer to slide off the forks. The trailer struck the employee across the shoulder and upper back, knocking him to the ground. He suffered fractured vertebrae and three broken ribs.
