Incident Narrative

An employee was moving a piece of 12-inch PVC pipe with a crane hoist. The hoist's slide rail broke, dropping the pipe. The pipe fell onto the employee's left hand, pinching his left ring and middle fingers against a steel rail. Both fingers were broken and severed; the middle fingertip was removed at the first knuckle.
