Incident Narrative

An employee was attempting to remove a piece of lathe from a jack ladder chain. When she pulled the lathe loose, the jack ladder chain started up and pinched the tip of her right middle finger between the chain lug and the metal plating. Her right middle finger was amputated at the first knuckle.
