Incident Narrative

An employee was operating an overhead crane inching a hoisted cylinder into place on a material handler with their left hand placed on the cylinder. When the hoisted cylinder slid into place on the material handler, the tip of the employee's left middle finger was pinched between the cylinder and the material handler's stabilizer bracket and amputated.
